Our Houston home is centrally located, close to downtown and the suburbs, so people can reach work, appointments and services without a car.

Residents get case management from day one: an assessment, a written plan they help build, and someone who checks in. We coordinate with medical, behavioral health and benefits providers so nothing falls through the cracks.

If there is a bed, we set up intake. You and your case manager write the plan together in the first week.

As soon as a bed is open

If we are not the right fit, we will tell you fast. If we are not the right fit, we will tell you fast and point you to someone who is. We never leave a person with nowhere to call.

How we measure

We count what actually matters

Not meals served. Not flyers handed out. We track how many people are still housed twelve months later, how many are working or in training, and how many keep the benefits we helped them get.

Those are the numbers a hospital discharge planner needs before they trust us with a patient — so those are the numbers we publish.
